1.Required Documentation
Send only the transcripts/documents indicated. Do not send any additionaldocumentation (letters of reference, certificates, achievements, financialstatements, etc.).

Interim and final transcripts should be submitted as soon as possible. Ifyour interim average is sufficiently high you may be considered for aconditional offer of admission. If your high school reports quarterly,transcripts from each quartile should be submitted. Generally speaking, it isinadvisable to request that a transcript be sent to McMaster before interimgrades are available if course work is in progress (ie. prior to aninstitution’s January grade release). Transcripts should show all gradesachieved plus all courses expected to be completed by the end of theacademic/school year.
All 105 applicants will receive their admission decision via e-mail. Afterapplying, please check your e-mail regularly for updates.

2.Submission of Documentation
It is the sole responsibility of the applicant to arrange for requireddocumentation to be delivered to McMaster University well in advance of anydeadline dates. Documentation received after the deadline for submission will bereviewed for admission on a case-by-case basis if space in the requestedprogram/faculty exists.

All documents submitted in support of your application for admission becomethe property of the University and are not returnable. If you are not accepted,or you fail to enroll following acceptance, your documentation will be destroyedat the end of the admission cycle. If you reapply, you must submit any newacademic information in addition to the documentation submitted previously.

When submitting your document(s), use the label(s) provided, with yourpreprinted student number on them. This will ensure that your documents arematched to your file in the most efficient manner possible. If you are unable touse the labels provided, you should ensure that all supporting documentation isclearly labelled with either your McMaster Student Number (found on yourconfirmation letter) or your Ontario Universities’ Application Centre (OUAC)application reference number.

3.Official Transcripts
An official transcript is a complete academic record of all individualcourses and grades achieved at an educational institution. It must be signed bya presiding official and bear the seal or certified stamp of the issuinginstitution. The signature, seal and/or stamp should appear on all pages of thedocument. A transcript is considered official only if it is sent directly toMcMaster from the issuing institution in a sealed envelope. The Universityreserves the right to require applicants with an English Language Proficiencyscore disparate from their academic English prerequisite achievement to presentfurther evidence of proficiency. If evidence of misrepresentation of fraudulentor falsified documentation is found, the penalty is severe. McMaster Universityreserves the right to rescind any Offer of Admission and/or scholarship.

Where documentation from a school outside of Canada is in a language otherthan English, a certified English translation (signed, sealed, and stamped) mustbe included with the original language documents. A certified translation mustbe issued by one of the following:

The translator must include a photocopy of the document(s) from which thetranslation was prepared, and an original statement indicating that thetranslation is accurate and authentic. The translator must also include theircontact information (including identification number and/or seal, name, address,and telephone number) printed name and signature in the statement.
